Who needs
the Vial of Life ?
People who are elderly or disabled and live alone.
People with a medical condition that might require the services of emergency medical personnel.
People with special or unusual medical conditions, which would require prompt identification if they are unconscious or unable to communicate in an emergency.
People who live alone and are mentally or physically unable to give necessary information to emergency medical personnel.
How do I use
my Vial of Life?
Fill out the medical information sheet and place it in the Vial of Life box. You might want to make a copy and give it to your physician or keep it in your wallet.
Place the Vial of Life decal on a front window or on your door where the emergency medical personnel will see it.
Fill out the registration form and return to Amador Senior Center. (A volunteer will call you periodically to remind you to update your information. You can request NOT to be called.)
Be sure to update your medications every time you have a change!
